Our specialized team of Hospice professionals works with the patient, family and attending physician to develop a plan of care that will best meet the patient's individual needs. Services include palliative care, pain and symptom management, psychological support, medical social work and spiritual/pastoral support.

For many people, their dream is to live the fullest life possible, for as long as possible, right up until a peaceful end at home where they are comfortable, free of pain and surrounded by the people and things they love. Heart of the Rockies Home Health and Hospice works to make that possible.

To facilitate treatment in the patient’s best interests, we have protocols for advance medical directives. These are documents signed by a competent person giving direction to healthcare providers about your future medical care should you become unable to make those decisions yourself.
