Free and Low-Cost Mammograms to Those Who Qualify

Mission Statement of the Chaffee County Mammography Fund:
To provide funds to pay for or defer the cost of a mammogram or alternative diagnostic procedure for the medically underserved, uninsurable, and underinsured people of Chaffee County and the surrounding communities.

You are at increased risk for breast cancer if any of the following applies to you:
You have a mother, father, sister, brother, daughter or son with breast cancer prior to age 50.
You have two or more 1st degree relatives (mother,father, sister, brother, daughter, son) with breast cancer at any age.
You have a personal history of breast cancer.

You may be eligible for payment of a mammogram with the Chaffee County Mammography Fund:
If your annual total gross income meets the financial guidelines established by the Colorado Dept. of Public Health and Environment and you cannot afford to pay for an annual mammography screening.
If you have received a recommendation from a medical professional for screening, diagnostic and/or ultrasound.
If you have received a prescription for a mammogram from your physician and payment for a mammography test has been denied by your insurance company.
Patients with Medicare Part B are not eligible.

If you think you may qualify for a free or low-cost mammogram, diagnostic, or ultrasound, please follow this procedure:
1. Applicant makes an appointment with HRRMC (Patient Financial Services Dept.,719-530-2200)to determine eligibility, fills out the application form, and provides proof of income level (pay stubs, Income Tax form, etc.).

2. Applicant/patient needs to have a prescription for the mammogram. A prescription may be obtained from one of the following:
His/her own physician
Planned Parenthood - Call 719-539-7291, 233 E. 2nd St., Salida, CO 81201

3. Once the applicant has a prescription and eligible determination, he/she will be issued a CCMF (Chaffee County Mammography Fund) voucher, and she/he may then make an appointment for a mammogram at the HRRMC Radiology Dept.

All 3 above steps can occur in one visit if you come in with proof of income and a prescription.

4. Patient presents HRRMC qualifying to hospital Admissions on the day of the appointment The results of the exam will be sent to the ordering physician, physician assistant, or nurse practitioner.

5. HRRMC will bill the CCMF for payment.

(The CCMF will use the current year's guidelines for services as established by the Federal Poverty Guidelines, up to 250% per household size, categories 1-3, but will, however, allow for exceptions determined on a case-by-case basis.)
